for older people in their last
phase of life

Our work is focused on older people living and dying well over a life long-lived.

Currently, older people with frailty too often fall between services either focused on living independently or imminently dying.

The PALLUP toolkit, created by practitioners, older people, unpaid carers and researchers, is a resource to support you and your team, to enact practice change in the care for older people with advancing frailty. Using the resources of the toolkit, you will be able to facilitate a half-day workshop, focussed on delivering collective service change for older people. Your team can be at any level of the system- the focus is the same- coming together with different perspectives to improve care.

The toolkit will help develop your understanding of the needs of older people with progressive frailty and provide you with a framework to develop your services to support a local change.
